How to get your maiden name back after divorce online in Ukraine
The Family Code says that a person who changed his surname upon marriage has the right, upon divorce, to either leave this surname, or return his premarital, maiden name.
Therefore, if you dissolve your marriage through the registry office, then in the application for divorce, you should indicate information that you intend to return your premarital surname after the dissolution of the marriage and information that your surname will be changed.
How to get your maiden name back after divorce in Ukraine
If you indicated information in the application for divorce in the registry office or the court made a positive decision in your favor regarding the dissolution of the marriage and change your surname, then you should apply to the registry office with a written application to change your surname. You should also attach a document of intent to change your surname.
This document may be a court decision on divorce, which will contain a request from you to change your surname to premarital. You should also add photos and pay a state fee.
The cost of the state fee is 5 hryvnia 10 kopecks. But, if you change your surname again, the cost will be 51 UAH.
The Civil Registry Office will consider your application within three months and after three months will make a positive decision and issue you a certificate of surname change.
After you receive a certificate of surname change, a note is put in your passport that your passport is valid for only one month and must be replaced due to the change of surname.
In this case, you need to change the passport of a citizen of Ukraine. Also, you should change other documents such as identification number, driver’s license, passport, etc.
That is, in fact, with a change in your surname, you will definitely have to change all documents.

- criminal proceedings have been opened against you or you are under investigation;
- you have outstanding or previously convicted;
- you have submitted inaccurate data;
- you are wanted;
- you have not submitted all the required documents;
- you are not a citizen of Ukraine.
